Measurements — Preliminary, External SLE, Posterior
Three tabs collect every measurement the tech or doctor takes during the exam.
Preliminary tab
The tech (or doctor) fills this before the doctor sees the patient:
- VA (Visual Acuity) — unaided and aided (with current Rx), OD / OS / OU, distance and near.
- Entrance GL / CL — the Rx the patient walked in with (glasses and/or contacts).
- Confrontation Fields — quick visual-field screening.
- Pupils — size, reactivity, APD.
- Color Vision — Ishihara or equivalent.
- NPC — near point of convergence.
- Amsler — macular grid test.
- Phorias / Vergences / Accommodation — binocular function.
- Iris Color · BP — quick notes.
- Autorefractor · Auto Keratometer — machine readings, entered manually or synced from the device.
Each subsection has its own note field for anything the numbers don't capture.
External SLE tab
Everything the doctor sees with the slit lamp plus IOP.
- SLE (Slit Lamp Exam) — lids/lashes, conjunctiva, cornea, AC, iris, lens.
- IOP / Tonometry — pressure readings with a small history chart to see the trend across visits.
- IOP Drops — which drops the patient is on for pressure.
- Gonioscopy — angle grading (for glaucoma workup).
- Visual Fields — VF perimetry results.
- Drawing — free-hand sketch pad for cornea/lid diagrams.
Posterior tab
Fundus (back of the eye):
- Cup — C/D ratio for each eye.
- Findings — everything else visible on dilated exam (vitreous, retina, macula, vessels, periphery).
- Medication · Info · Note — ancillary fields.
Tips
- All three tabs auto-save. You don't need a Save button.
- If a section is normal, leave it blank or tick the "normal" checkbox where present — no need to type "WNL" everywhere.
- Some fields have small drawing icons — those open a mini-diagram tool.
